How This Calculator Works
This calculator estimates your cardiovascular (aerobic) endurance using the Balke Protocol, a treadmill test developed by Dr. Bruno Balke in the 1950s for U.S. Air Force fitness assessment. Unlike the Bruce Protocol, which increases both speed and incline every 3 minutes, the Balke test holds speed constant and increases only the incline, in small, frequent steps. You walk until volitional exhaustion, and your total time converts directly into an estimated VO2max.
Read This Before Attempting the Test
Like the Bruce Protocol on this site, the Balke test is a maximal test — performed to true exhaustion, not a sub-maximal effort with extrapolation. The difference between the two is in how you get there: Balke's constant speed and small, steady grade increases are considered gentler and more predictable than Bruce's larger jumps in both speed and incline every 3 minutes, which is exactly why Balke is frequently the preferred choice for cardiac rehabilitation, older adults, and deconditioned individuals in clinical settings. "Gentler" does not mean "risk-free."
- This is still a test to volitional exhaustion. It should not be self-administered by anyone with known heart disease, chest pain, uncontrolled high blood pressure, or other cardiovascular risk factors without a physician's explicit clearance and, ideally, direct supervision.
- Stop immediately for chest pain, severe or unusual shortness of breath, dizziness, lightheadedness, or any other concerning symptom.
- Because the standard protocol has no built-in warm-up (the gradual grade increase serves that role), consider a brief separate warm-up if you have any doubt about starting cold.
Step 1: Enter Your Details
- Sex — the formula, and the protocol itself, differ by sex.
- Age — used to determine your classification standards (the VO2max formula itself does not include an age term).
- Total Time to Exhaustion — the total time on the treadmill, in minutes and seconds, from the start of the test to the point you can no longer continue.
The Test Protocol
The standard Balke protocol (the version these formulas were derived from) differs by sex:
Women: Constant speed of 3.0 mph, starting at 0% grade, with the incline increased by 2.5% every 3 minutes until exhaustion.
Typical test duration is 9–15 minutes; because the test starts gently and gradually, no separate warm-up period is built into the standard protocol.
A Real Caveat: Which "Balke Protocol" Did You Do?
Unlike the Bruce Protocol, which has one dominant, standardized version used almost everywhere, "the Balke test" refers to a family of related but distinct protocols that vary in starting speed, grade increment size, and timing between increases. You may encounter variants that increase grade every 2 minutes instead of every minute, that individualize starting speed to reach a target heart rate first, or that use different speeds entirely (the Modified Balke and Naughton protocols, among others, are specifically designed to be even gentler for severely deconditioned populations).
The VO2max formulas below are only accurate for the specific protocol described above — the one Pollock's research team used when they derived these equations. If you or your gym uses a different Balke variant, your total time isn't directly comparable to what these formulas expect, and your VO2max estimate will be off. This is a genuine, documented limitation of time-based Balke formulas in general, not something specific to this calculator.
Step 2: Estimating VO2max
Total time to exhaustion goes directly into one of two sex-specific linear equations:
Women (Pollock et al., 1982): VO2max (ml·kg⁻¹·min⁻¹) = 1.38T + 5.22
where T is total time in minutes (e.g., 10 minutes 30 seconds = 10.5). Both equations come from Michael Pollock's research comparing multiple treadmill stress test protocols against directly measured VO2max, the same research group and era that produced the female Bruce Protocol equation used elsewhere on this site.
METs
METs (metabolic equivalents) are VO2max divided by 3.5, the oxygen cost of sitting quietly at rest — a simple secondary readout sometimes used directly in clinical settings alongside VO2max.
Step 3: Classifying Your VO2max
Your VO2max is classified using the same source as the other VO2max-based calculators here: general adult VO2max norms from the Cooper Institute's Aerobics Center Longitudinal Study, as published in ACSM's Guidelines for Exercise Testing and Prescription (11th ed., 2021, Table 4.7), merged into this site's standard five tiers using only ACSM's own published boundaries.
The Smooth Age Model, Table, and Chart
Classification standards use the same age-bracket structure as the other VO2max-based calculators here: anchored at 24.5/34.5/44.5/54.5/64.5/75, interpolated smoothly, with results past age 75 extrapolated beyond ACSM's published "70+" bracket. The table lists minimum VO2max per tier by age bracket; the chart plots the five tiers as bands across ages 18–89, with your result marked as a dot.
Fitness Age and Percentile
Your Fitness Age is the age at which your estimated VO2max would be typical (mid-Average-band) performance. The percentile is a modeled approximation mapping the tier boundaries to assumed percentile positions, the same approach used for the other ACSM-classified calculators here.
Balke vs. Bruce: Why Choose One Over the Other
Both protocols estimate the same underlying quantity and correlate well with each other, but they suit different people. Bruce's larger jumps in speed and grade get most people to exhaustion faster (commonly 6–15 minutes), which is efficient for testing large numbers of relatively healthy people. Balke's smaller, more frequent grade increases at a constant, comfortable walking speed make it more tolerable for people who might struggle with Bruce's faster stages — at the cost of a longer test. Neither is more "accurate" in principle; the choice is mostly about which progression suits the person being tested.

Limitations and Important Caveats
- This is a maximal-effort exercise test. Please take the safety guidance above seriously, particularly if you have any cardiovascular risk factors.
- Protocol variant matters more here than for Bruce. As explained above, "the Balke test" isn't a single standardized protocol the way Bruce's is. Using a different grade-increment timing or starting speed than the one these formulas were derived from will introduce error beyond what's typical for the other calculators on this site.
- No age term in the VO2max formula. Like the Queens College Step Test and Bruce Protocol, this equation predicts VO2max from time alone; age is used here only to select your classification bracket.
- Motivation and pacing affect results. Because this is a true maximal test, your result partly reflects your willingness and ability to push to genuine exhaustion, not fitness alone.
- Treadmill calibration matters. An improperly calibrated treadmill will distort your true time and therefore your VO2max estimate.
- Approximate percentile. As with the other VO2max-based calculators here, the percentile is a modeled approximation, not an exact population statistic.
